We recommend always downloading​ the forms directly from our website rather than reusing previously saved versions. This will ensure you’re always using the most up-to-date forms.

This page offers a collection of forms for refugee claims, appeals, immigration appeals and detentions that will allow​ you to complete your process.

Most of the forms available on this site are PDF forms that can be completed directly on-screen. Forms requiring signatures must be printed. Print or save the forms and then submit them following the instructions found on the forms.

Refugee appeals

The RAD is temporarily not requiring signatures on documents and RAD forms submitted in support of an appeal in order to expand the use of electronic communication with the RAD and promote physical distancing.​

Refugee claims

Immigration appeals

  • Notice of disclosure form

    When providing documents to the Immigration Appeal Division (IAD), appellants or their counsel are required to provide a written statement indicating how and when they were provided to the Minister's counsel.

  • List of witnesses form

    Appellants or their counsel are required to provide a list of any person (including you) who testifies at a hearing with the Immigration Appeal Division (IAD).

  • List of documents form

    If more that one document is being submitted to the Immigration Appeal Division (IAD), appellants or their counsel are required to provide a list of such documents.

  • Notice of appeal – removal order appeal

    This Notice of Appeal form is required to appeal a removal order received at an admissibility hearing or at an examination.

  • Notice of appeal – residency obligation appeal

    This Notice of Appeal form is required to appeal a decision made outside of Canada that a permanent resident did not meet their residency obligation.

  • Notice of appeal – sponsorship appeal

    This Notice of Appeal form is required to appeal the refusal of an application for a permanent resident visa for a sponsored family member.

  • Confirmation of Hearing Needs

    This Confirmation of Hearing Needs form may be used to inform the IAD of interpreter and witness needs for a hearing.

  • Notice of Withdrawal of Appeal

    The Notice of Withdrawal form may only be used if no substantive evidence has been accepted in the hearing of the appeal.
